Job summary

MEP Health is seeking an RN Nurse Navigator to guide patients and families through the full care continuum. You will coordinate services, educate patients, and collaborate with multidisciplinary teams to deliver seamless, compassionate care.

In this role you will navigate care transitions, connect patients to resources, advocate for patient-centered plans, and support families throughout treatment decisions.

Qualifications

• Current RN license and strong clinical knowledge.
• Excellent communication, education, and interpersonal skills.
• Ability to coordinate care across multiple disciplines.
• Compassionate, patient-focused approach and strong advocacy mindset.
• Organizational skills to manage care coordination effectively.

Responsibilities

• Navigate patients through care transitions and connect them to appropriate resources.
• Coordinate with healthcare teams to support continuity of care.
• Provide education, emotional support, and advocacy to help patients understand treatment plans.
• Help remove barriers to access and ensure patients receive the right services.
• Maintain patient-centered communication with families and care partners.
