Job summary

Landmark Hospital of Columbia is seeking a Clinical Navigator (RN) to guide patients and families through complex care plans and the healthcare system. You will act as the primary contact for referrals, business development, and community outreach, coordinating with providers, patients, and families to ensure seamless care.

The role requires strong communication, public speaking, and strategic outreach; travel within the territory is expected, with daily coordination to improve patient outcomes

Qualifications

• Active RN license required or pending license in good standing.
• Education: Associate's degree is required; Bachelor’s degree preferred.
• Experience in patient advocacy, health education, case management, or clinical/hospital setting.
• Minimum of 3 years of outside sales experience in specialty hospitals, hospice, or similar healthcare setting.
• Strong communication, public speaking, and relationship-building skills.

Responsibilities

• Coordinate referrals with hospital clinical and case management teams for LTAC admissions.
• Serve as liaison between patients, doctors, social workers, and administrative staff.
• Educate patients and families about diagnoses, treatment options, and care pathways.
• Identify and develop referral sources to drive patient admissions.
• Represent Landmark Hospital services to staff, physicians, hospitals, patients, and families.
