Job summary

Southcoast Health in New Bedford, MA seeks an RN-Care Navigator to coordinate care across the continuum for patients in the Care Center. Required: BSN or equivalent, MA/RI RN license, 4 years clinical experience, BLS; Oncology/GYN experience preferred.

Qualifications

• BSN preferred or equivalent, and RN licensure in MA/RI.
• 4 years of clinical experience required, ambulatory/outpatient experience preferred.
• Previous training as a care navigator, coordinator, or manager is highly desirable.

Responsibilities

• Coordinate care across the continuum for assigned patients within the Care Center.
• Assess physical, psychological, and social needs; organize and facilitate clinical visit needs.
• Educate patients and families; provide education materials and connect to resources.
