Job summary

Medical City McKinney is seeking an RN Case Manager to lead patient care coordination within a fast-paced hospital setting in Texas. You will assess psychosocial, medical and discharge needs, manage length of stay, and develop a safe, appropriate discharge plan in collaboration with the care team.

In partnership with Social Services and physicians, you will drive the plan of care, monitor progression using InterQual criteria, refer to payer programs when needed, and advocate for patients' rights

Qualifications

• RN with TX license required.
• Associate Degree in Nursing or Nursing diploma required; Bachelor's preferred.
• 3+ years hospital nursing experience or 2+ years case management experience.
• Certification in case management and InterQual experience preferred.

Responsibilities

• Perform comprehensive psychosocial, medical and discharge assessments.
• Reassess patient’s condition and mitigate readmission risk with education and resources.
• Coordinate plan of care and drive discharge planning with the multidisciplinary team.
• In partnership with Social Services, ensure post-acute medical needs and level of care are appropriate.
• Refer to third-party payer, disease and case management programs as needed.
• Document care coordination interventions and communicate with the health care team.
• Advocate for patient rights and culture-sensitive care.

Education

Associate Degree in Nursing or Nursing diploma

Bachelor's Degree in Nursing (preferred)
