Methodist Hospital Northeast in Texas is seeking an experienced Case Manager to support patients and families through care transitions. You will coordinate services, develop individualized care plans, and collaborate with physicians, nurses, and leaders to ensure safe, quality results.

The role emphasizes compassionate, patient-centered care and adherence to hospital standards. Applicants should be a graduate of an accredited nursing program with Texas RN licensure; preferred supervisory CM

Qualifications

• Graduate of an accredited nursing program (Diploma/AA/BS).
• Minimum 3 years Case Management experience or 2 years CM + Nurse Manager.
• Current Texas RN licensure.
• Preferred: Case Management supervisory experience.

Responsibilities

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers.
• Develop individualized treatment plans reflecting patient goals.
• Complete psychosocial assessments and document findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team.
• Promote hospital values of compassion, respect, and excellence.
