Job summary

Medical City Heart & Spine Hospitals in Dallas, TX is seeking an RN Case Manager for a full-time day shift. You will coordinate care, assess discharge needs, and collaborate with the multidisciplinary team to ensure efficient patient transitions.

The role requires TX RN licensure, an ADN or nursing diploma (BSN preferred), and 3+ years of hospital nursing or 2+ years in case management. Certification and InterQual experience are preferred.

Qualifications

• Registered Nurse with current TX state license required.
• Associate Degree in Nursing or Nursing diploma required; Bachelor’s Degree in Nursing preferred.
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required.
• Certification in case management preferred.
• InterQual experience preferred.

Responsibilities

• Performs comprehensive assessment of psychosocial, medical and discharge needs of patients/families.
• Reassesses patient condition and readmission risk; educates on resources and preventative care.
• Coordinates plan of care and drives discharge planning with multidisciplinary team.
• Ensures post-acute medical needs and level of care are appropriate in partnership with Social Services.
• Refers patients to third party payer, disease and case management programs as needed.
• Facilitates patient throughput and effective care transitions.
