Qualificações

Responsabilidades

• Leads multidisciplinary team to develop and communicate plan of care for defined patient population.
• Implements clinical pathways and education needs for patients/families.
• Coordinates care transitions and communicates with providers for seamless handoffs.
• collaborates with social work, utilization review, and others to maximize resources.
• Coordinates patient throughput and discharge planning
• Performs daily rounds to ensure discharge readiness and patient/family preparation.
