Job summary

Duke Health Lake Norman Hospital in Mooresville, NC, is seeking a Registered Nurse Care Coordinator to integrate with the health care team and coordinate patient care across inpatient, outpatient, and home health settings.

The ideal candidate holds a BSN, has at least 3 years of nursing experience, and can serve as a clinical expert for complex populations, guiding families and care teams through transition and discharge planning.

Qualifications

• Knowledge of scope for a registered nurse.
• Knowledge of and appropriate application of the nursing process.
• Ability to independently seek out resources and work collaboratively.
• Ability to establish and maintain effective working relationships.
• Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others.
• Ability to teach patients and families in accordance with the nursing plan of care.

Responsibilities

• Collaborates with providers, case managers, and health care team to assess patient needs and plan care.
• Provides triage support for patient populations across clinical areas.
• Monitors patient outcomes and documents responses to care.
• Coordinates prescription refills and prior authorizations.
• Completes clinical paperwork and forms as required.
• Coordinates complex patient visits and pre-visit assessments.
• Participates in QAPI activities and quality improvement.
• Educates patients and caregivers about services, disease states, and interventions.
• Assists in discharge planning and transition to hospital care coordination.
• Provides community resources and referrals.
