Job summary

Duke University Hospital in Durham, NC is seeking a Nurse Navigator to coordinate patient care across oncology services and transitions. The RN will advocate for patients, educate families, and partner with clinicians to ensure timely access and seamless care across the continuum.

As an experienced RN, you will document encounters in EPIC, promote multidisciplinary collaboration, and help patients navigate complex treatment journeys while maintaining confidentiality and compassion.

Qualifications

• BSN required.
• Current NC RN licensure (or compact) and BLS.
• Three years of oncology experience; nurse navigation preferred.
• Completion of national nurse navigation training preferred.

Responsibilities

• Coordinate multidisciplinary care across the care continuum.
• Provide communication, advocacy, and education in a culturally competent manner.
• Efficiently triage new patient referrals to ensure timely access to care.
• Collaborate with care team and facilitate sequence of patient workup and survivorship.
• Document all patient encounters in EPIC and update the care plan.
• Promote transitions in care and advanced care planning as needed.
