Job summary

Qualifications

• BSN degree required.

Responsibilities

• Collaborate with providers and care team to assess patient needs and plan care.
• Provide triage support across clinical areas.
• Coordinate discharge planning and post-hospitalization care.
• Educate patients and families on treatments and care plans.
• Handle prescription refills and prior authorization forms.
• Maintain timely documentation and participate in QA/PI activities.
