Job summary

Qualifications

• Currently licensed as a Registered Nurse by the North Carolina Board of Nursing. BSN preferred.
• BLS Certification through American Red Cross or American Heart Association
• Minimum of 2 years of clinical experience in a hospital setting.
• Previous utilization and/or discharge planning experience preferred.
• Excellent organizational and communication skills required.

Responsibilities

• Coordinate patient care through multidisciplinary collaboration.
• Assist with planning patient care including progression and discharge planning.
• Ensure effective use of hospital and community resources.
• Monitor patient outcomes through concurrent utilization review and discharge planning.
