Qualifications

• BSN required and NC RN licensure or compact licensure.

Responsibilities

• Coordinate home health, hospice, and infusion services for patients at Duke University Hospital.
• Educate patients and caregivers to ensure safe home care and discharge planning.
• Document all activity in Maestro and CareTend and other systems.
• Maintain communication with the health care team regarding assessment findings and discharge needs.
• Assist with site-specific duties and after-hours coverage as designated.
