Job summary

Duke Health Lake Norman Hospital in Mooresville, NC seeks a Nurse Case Manager to coordinate discharge planning across the continuum of care, ensuring CMS compliance and optimal use of resources. You will assess, plan, implement and evaluate care transitions with the health care team.

Requirements include a BSN or MSW, 3 years of relevant experience, and current NC RN licensure (or MSW with eligibility). Case Management Certification is required within 3 years of hire.

Qualifications

• 3 years of relevant case management experience.
• Current RN licensure in NC or MSW with licensure eligibility if no RN.
• Requires Case Management Certification (ACM, CCM or ANCC) within 3 years of hire.

Responsibilities

• Coordinate and complete discharge planning across the continuum of care.
• Maintain communication with care teams on assessment, discharge planning and provider orders.
• Ensure compliance with CMS and other review agencies and document timely actions.
