Job summary

• Document activity in Maestro and CareTend and related systems.
• Engage with referral sources and community partners to ensure seamless transitions.
• Maintain knowledge of insurance benefits and regulatory guidelines.
• Perform site-specific duties and after-hours coverage as designated.
