Job summary

Molina Healthcare is seeking an RN Care Manager to provide coordination of LTSS and care management activities for high-need members in Illinois. You will collaborate with an interdisciplinary team to assess needs, develop and monitor care plans, and ensure timely waiver processes and service delivery across settings.

Travel of 25-40% may be required to meet state contract requirements. The ideal candidate has an active RN license, 2+ years of care management experience, strong communication

Qualifications

• At least 2 years of health care experience with care management/LTSS exposure.
• RN license active and unrestricted in state of practice.
• In some states, a bachelor's degree in a health care related field may be required.
• Valid driver's license, reliable transportation for job-related travel.
• Ability to work independently and manage multiple projects.

Responsibilities

• Complete comprehensive member assessments within timelines and may require in-person home visits.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members and care network.
• Promote integrated care across behavioral health, LTSS, and community resources.
• Assess barriers to care and facilitate access to services and funding sources.
• Travel 25-40% based on state requirements.
