Job summary

Qualifications

• RN license active and unrestricted in state of practice.
• Bachelor's degree in a health care related field may be required depending on state/contractual requirements.
• Valid driver’s license and reliable transportation for job-related travel.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers and health professionals.
• Monitors care plans and adjusts interventions as needed.
• Coordinates services across behavioral health, LTSS, and community resources.
