Qualifications

• At least 2 years of experience in health care with 1 year in care management and LTSS exposure.
• RN license active and unrestricted in state of practice.
• Bachelor's degree may be required in some states per contract rules.
• Valid driver's license and reliable transportation for travel requirements.
• Ability to work independently and in diverse settings with focus on outcomes.

Responsibilities

• Completes comprehensive member assessments within required timelines, including home visits.
• Facilitates waiver enrollment/disenrollment processes and develops care plans.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Coordinates services across behavioral health and LTSS with care team.
• Assesses medical necessity and authorizes relevant waiver services.
• Leads ICT meetings for service authorization decisions and collaboration.
• Educates and motivates members using motivational interviewing techniques.
• Address barriers to care and supports psycho/social/financial needs.
