Qualifications

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with member and care network.
• Monitor care plans and adjust as needed to achieve goals.
• Integrate behavioral health and LTSS services for continuity of care.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings for approvals.
• Provide motivational interviewing and education to members.
• Identify barriers and assist with psycho/social, financial, and medical obstacles.
• Travel locally 25-40% as required.
