Job summary

Molina Healthcare seeks a Registered Nurse to support care management and LTSS activities in Illinois. You will collaborate with a multidisciplinary team to coordinate member care across the continuum, aiming for high-quality, cost-effective outcomes.

You will assess members, develop and monitor care plans, facilitate waiver enrollment, and coordinate services with caregivers, physicians, and networks. Travel up to 40% may be required; strong communication and organizational skills are essential.

Qualifications

• Registered Nurse (RN) with an active license in the state of practice.
• Minimum 2 years in health care with 1 year in care management or LTSS.
• Experience with disability, chronic conditions, SUD, or LTSS preferred.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor and adjust care plans to achieve goals and meet needs.
• Promote integration of services across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Collaborate with interdisciplinary care teams for approval/denial of services.
