Job summary

Molina Healthcare is seeking an RN to support care management and LTSS activities in Illinois. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and drive outcomes.

Qualifications

• At least 2 years of health care experience with at least 1 year in care management or related field.
• RN license must be active and unrestricted in the practice state.
• Bachelor's degree in a health care field may be required depending on state.
• Valid driver's license, reliable transportation, and auto insurance for travel.
• Ability to work independently, manage multiple projects, and communicate clearly.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, and providers.
• Monitor and adjust care plans based on progress and barriers.
• Promote integrated LTSS and behavioral health services for continuity of care.
• Assess medical necessity and authorize appropriate waiver services.
