Job summary

Qualifications

• At least 2 years of healthcare experience with at least 1 year in care management or LTSS.
• RN license active and unrestricted in state of practice; valid driver’s license.
• May require Bachelor's degree depending on state/contractual requirements.
• Ability to work independently and coordinate with diverse teams.
• Strong verbal and written communication; proficient with standard software.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits.
• Develops and implements care plans including waiver services with teams and members.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Coordinates LTSS and behavioral health services to ensure continuity of care.
• Facilitates ICT meetings to approve or deny services and communicates decisions.
• Travels locally 25-40% as required by state/contractual requirements.
