Job summary

Molina Healthcare is seeking a Nurse Care Manager to support care coordination for LTSS across managed care settings. You will collaborate with an interdisciplinary team to develop and implement care plans that address member needs and goals.

The role requires travel (25-40%), in-state visits, and strong communication skills to engage members, caregivers and healthcare professionals while ensuring quality and cost-effective care.

Qualifications

• RN license active in state of practice.
• At least 2 years in health care including care management or LTSS.
• Experience with persons with disabilities, chronic conditions, or substance use disorders is a plus.
• Ability to operate proactively and work independently.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians and support network.
