Qualifications

• RN license active in state of practice.
• 2+ years in health care with care management/LTSS exposure.
• Willingness to travel 25–40% locally.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and care team.
• Monitors care plan effectiveness and documents outcomes.
• Promotes integration of LTSS and behavioral health services for continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Coordinates ICT meetings for service approvals/denials.
• Educates and motivates members during contacts using motivational interviewing.
• Identifies barriers to care and coordinates support to address psycho/social/financial/medical obstacles.
• May provide consultation and resources to peers as needed.
• Travel (25–40%) may be required.
