Job summary

Molina Healthcare in Illinois seeks a Care Manager RN to support LTSS activities and coordinate member care across the multidisciplinary team. You will help ensure progress toward outcomes and contribute to quality, cost-effective member care.

The role requires completing assessments, developing care plans, monitoring effectiveness, and coordinating waiver services while collaborating with members, caregivers, physicians, and care teams. Travel as needed is anticipated.

Qualifications

• RN license active and unrestricted in state of practice.
• 2+ years health care experience including care management/managed care.
• Experience with persons with disabilities and LTSS preferred.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Develop and implement care plans with members, caregivers, physicians and support network.
• Monitor care plan effectiveness and document interventions/goal achievement.
• Promote integration of LTSS and behavioral health services and community resources.
• Identify barriers to care and provide coordination to address psycho/social/financial/medical needs.
• Travel 25-40% as required by state/contractual requirements.
