Molina Healthcare is seeking a qualified RN Care Manager to support LTSS and care coordination. You will collaborate with multidisciplinary teams to deliver integrated member care and ensure progress toward outcomes.

The role involves conducting home visits, coordinating waiver enrollment, and developing effective care plans with members, caregivers, and healthcare professionals. Travel to member sites is anticipated.

Qualifications

• RN with active license in state of practice.
• Minimum 2 years in health care; 1 year care management/ LTSS experience.
• Valid driver''s license and reliable transportation for travel.
• Ability to work independently and communicate effectively.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops care plans with members and care network to address needs and goals.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
