Job summary

Responsibilities

• Coordinate patient referrals for home health, hospice, and infusion services.
• Educate patients and families on home care services and infusion devices.
• Maintain documentation in Maestro and CareTend and other systems.
• Communicate findings and discharge plans with the care team to ensure seamless transitions.
• Develop and maintain relationships with referral sources and internal/external customers.
