Job summary

Duke Health is seeking a Care Coordinator to assess and coordinate home health, hospice, and infusion services for hospitalized, clinical/outpatient, and home-based patients in the Durham region. The RN will educate patients and caregivers on home infusion devices and work within an interdisciplinary team to discharge planning and community education.

Responsibilities include daily patient/family education, coordinating with Case Managers and physicians, and documenting activities in Maestro,

Qualifications

• BSN is required and RN licensure in NC or compact licensure in participating states.
• BLS certification is required and may be renewed during orientation.
• Three years of nursing experience are required.
