Job summary

The Duke HomeCare & Hospice Care Coordinator coordinates home health, hospice, and infusion services across Duke Health facilities, including inpatient and outpatient settings. You will educate patients and families about infusion devices and work with the interdisciplinary team to ensure seamless care.

Required BSN, NC or compact RN licensure, and BLS, with at least three years of nursing experience. Duties include electronic documentation and rotating shift coverage across Duke sites.

Qualifications

• BSN required; RN licensure in North Carolina or compact licensure is required.
• BLS required; can be renewed during orientation.

Responsibilities

• Assess patient eligibility for home care and initiate appropriate referrals.
• Coordinate infusion services for adult and pediatric patients.
• Educate patients and caregivers on infusion devices and home care processes.
• Maintain documentation in Maestro, Netsmart, HCN360 and other systems.
• Collaborate with hospital case managers and referral sources to optimize discharge planning.
