Duke HomeCare & Hospice is seeking an RN Coordinator to support home health, hospice, and infusion services with weekend coverage at Duke University Hospital. The role focuses on coordinating patient referrals and educating patients and families in home care settings.

The ideal candidate holds a BSN, NC RN license, and BLS certification, with at least three years of nursing experience. This position requires strong collaboration across multidisciplinary teams and a commitment to seamless care

Qualifications

• BSN required; RN licensure in NC (or compact) required
• BLS required; may be renewed during orientation
• Three years of nursing experience required

Responsibilities

• Coordinate patient referrals at Duke University Hospital on weekends; support home health, hospice, and infusion services.
• Assess home care needs and educate patients and families on home care and infusion devices.
• Maintain documentation in Maestro and CareTend; ensure timely discharge planning and referrals.
• Communicate with interdisciplinary teams and referral sources to improve care transitions.
• Develop and maintain relationships with internal and external customers and participate in after-hours coverage if needed.
