Job summary

Duke HomeCare & Hospice seeks a Care Coordinator to coordinate home health, hospice, and infusion services for hospitalized, clinic/outpatient, and community patients across Duke Health facilities, including the inpatient hospice in Durham, NC.

The RN will assess admission information, guide discharge planning, educate patients and caregivers on infusion devices used at home, and collaborate with a multidisciplinary team to ensure seamless care and effective communication with referral sources.

Qualifications

• BSN required; current RN licensure in NC or compact state
• BLS certification required; renew during orientation if not current
• Three years of nursing experience required

Responsibilities

• Assess patient eligibility for home care, initiate the appropriate home care plan and educate patient/family
• Identify barriers during education and coordinate with Case Manager/physician team for discharge planning
• Provide ongoing daily assessment and monitor referrals until discharge and coordinate with hospital Case Manager
• Communicate findings with health care team and update care plan as needed
• Document all activity in Maestro, Netsmart, HCN360 and other systems
• Meet referral sources regularly to improve referral processes and identify education opportunities
• Maintain knowledge of insurance benefits, reimbursement guidelines and their impact on services
• Know regulatory guidelines related to home care services
• Maintain knowledge of DHCH services, products and supplies
• Maintain referral source contacts and analyze referral trends and needs
• Collaborate with referral center, finance, and home program to prevent insurance denials
• Develop positive relationships with internal and external customers
• Job responsibilities may include site-specific duties, weekend/holiday coverage
• Perform other related duties as needed
