Duke HomeCare & Hospice is seeking a dynamic Care Coordinator to coordinate home health, hospice, and infusion services for patients at Duke University Hospital and occasionally at Duke Regional Hospital. You will educate patients and families, support discharge planning, and help ensure seamless transitions from hospital to home.

The role requires BSN and NC or compact licensure, with daily/weekday hours and on-call coverage as needed.

Qualifications

• BSN required.
• Current RN licensure in North Carolina or compact state.
• BLS required.

Responsibilities

• Coordinate home health, hospice, and infusion services for patients.
• Educate patients and caregivers on home care needs and discharge plans.
• Maintain documentation in CareTend/Maestro and other systems.
• Collaborate with interdisciplinary teams and referral sources to ensure high-quality transitions from hospital to home.
