Job summary

Duke Health's HomeCare & Hospice division is seeking a Care Coordinator in Durham, NC. The role focuses on coordinating home health, hospice, and infusion services for patients at Duke University Hospital with occasional rotations to Duke Regional Hospital.

You will educate patients and families and ensure safe, seamless care transitions, including documentation and interdepartmental collaboration. The schedule is Monday–Friday, 8:30 AM–5:00 PM, with an on-call rotation every 4–6 weeks.

Qualifications

• BSN required and NC RN licensure (compact licensure accepted).
• BLS required; renewal during orientation.
• Three years of nursing experience preferred.

Responsibilities

• Coordinate home health, hospice and infusion services for patients.
• Assess home care needs and educate patients and families; develop discharge plans.
• Document activities in Maestro, CareTend and related systems.
• Communicate with hospital Case Manager and interdisciplinary teams to ensure smooth transitions.
• Maintain knowledge of insurance benefits, reimbursement guidelines, and regulatory requirements.
• Participate in on-call rotation every 4–6 weeks.
