Job summary

WellMed, part of the Optum family, is seeking a RN to join our team in Dallas, TX. The Healthcare Manager will support patients with high risk health conditions to navigate the healthcare system and act as an educator, resource, and advocate for patients and families.

The role collaborates with clinicians, nurses, pharmacists, and social workers, performing assessments, med reconciliation, education, and referrals while documenting in EMR and supporting transitions in care as needed.

Qualifications

• RN licensed in the state of employment with a nursing degree.
• Bachelor's or Associate degree with 4+ years of experience.
• Current BLS certification.
• 2+ years in a physician's office, clinical, or hospital setting.
• Experience with chronic diseases and care coordination.
• Proficient with EMR systems.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Coordinate longitudinal care with assessments, med rec, motivational interviewing, education, and referrals.
• Support transition of chronic patients from inpatient to outpatient care with assessments and contingency planning.
• Establish or update care plans with providers.
• Document accurately in the EMR and participate in PCC meetings.
• Prepare reports for weekly meetings and maintain nursing competencies.
