Job summary

WellMed, part of the Optum family, seeks a Healthcare Manager to support patients with high-risk conditions and navigate the healthcare system. The role emphasizes education, advocacy, and collaboration with physicians, nurses, pharmacists, and social workers to optimize quality of life.

In this self-directed environment, you will perform nursing duties within the RN scope of practice and engage in care coordination, documentation, and continued professional development.

Qualifications

• RN with active, unrestricted license in state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical, or hospital setting.
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD).
• Proficient with electronic medical records (EMR).
• Strong verbal and written communication; solid interpersonal skills.
• Ability to interact productively with individuals and multidisciplinary teams.
• Strong organizational and prioritization skills.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care: assess health, reconcile medications, coach on self-management, educate patients, refer to resources.
• Assist in transition from inpatient to outpatient care: assess needs, reconcile meds, plan contingencies, educate patients, arrange post-discharge needs.
• Coordinate with providers to update care plan.
• Document accurately in the EMR and participate in PCC meetings.
• Prepare reports for weekly meetings and maintain clinical competence.
