Community Health & Wellness Center in Torrington, CT is seeking an RN Navigator/ACO Specialist to provide patient-centered clinical navigation, care coordination, and case management to access appropriate care and improve outcomes.

The RN will collaborate with physicians, nurses, health plans, and community resources, educate patients, close care gaps, and support population health initiatives within CT statutes and ACO programs.

Qualifications

• RN with CT license and valid nursing status.
• BSN preferred; ability to work across care teams.
• Experience with care coordination and case management desirable.

Responsabilités

• Serve as primary navigator and resource for patients and care team.
• Coordinate transitions of care across hospital, primary and specialty care.
• Identify gaps in care and connect patients with resources.
• Educate patients and caregivers on services and preventive care.
• Collaborate with health plans and community organizations to support program goals.
• Maintain accurate documentation in EHR and data reporting.
