Job summary

Access Community Health Network is seeking a Care Coordinator to lead care navigation across transitions, education, and coaching for patients. The role supports both RN and Social Worker tracks, with licensure in Illinois and clinical experience required.

The position emphasizes team leadership, use of EMR, and patient empowerment through structured care planning and education across the care continuum.

Qualifications

• Must have Illinois license: LCPC, LPC, LCSW or LSW for Social Worker track.
• RN track requires Illinois RN licensure and an associate degree (bachelor’s preferred).
• Minimum 2 years (Social Worker path) or 1 year (RN path) clinical experience.

Responsibilities

• Lead intake process and care plan development.
• Coordinate patient navigation across care transitions.
• Provide health education, advocacy and coaching.
• Conduct face-to-face visits per policy and workflow.
• Support families in self-management and goal achievement.
• Ensure post-emergency department and hospital follow-up.
