CHRISTUS Health is seeking an RN Navigator for Population Health to coordinate care across the healthcare continuum and improve health outcomes. You will work with ACO/CIN providers, patients, and families to develop individualized care plans and ensure a well-organized, safe plan of care.

You will manage length of service and promote efficient resource use while maintaining accurate documentation in the EHR and adhering to quality and compliance standards.

Qualifications

• Bachelor’s Degree in Nursing required or preferred.
• 3 years of clinical experience.
• 2 years of case management experience.
• Knowledge of population health management principles.

Responsabilités

• Care coordination for complex patients; discharge planning and transitions.
• Educate patients on medications and follow-up care; promote preventive care.
• Perform comprehensive patient assessments to identify needs and barriers.
• Develop individualized care plans based on assessments and guidelines.
• Advocate for patients and help navigate the healthcare system.
• Collaborate with physicians, social workers, and community resources.
• Monitor changes and communicate updates to PCPs and specialists.
• Document all patient interactions and outcomes in the EHR.
• Participate in quality improvement initiatives to enhance care.
• Ensure compliance with regulatory requirements and organizational policies.
