Job summary

You will liaise with patients, caregivers, providers, and payors to promote timely access, effective transitions of care, and continuous improvement in population health outcomes. Qualifications

• Graduate of an accredited Nursing Program.
• Experience with electronic platforms and portals to monitor care gaps.
• Knowledge of managed care and population-based disease management concepts.

Responsibilities

• Serve as primary contact and clinical resource for patients and care team.
• Assess patient needs and develop individualized care plans.
• Coordinate continuity of care and transitions across settings.
• Educate and support patients and caregivers to make informed health decisions.
• Document in EHR and review data to close gaps in care.
• Collaborate with physicians and other health professionals to promote quality, cost-effective care.
