Responsibilities include coordinating care across the continuum, promoting patient engagement and health literacy, and ensuring HIPAA compliance while driving quality improvement and effective use

Qualifications

• Bachelor’s degree in Nursing (BSN) required.
• Five years of clinical or outpatient experience preferred.
• Active RN license in South Carolina.
• Proficient with EMR systems and basic computer skills.

Responsabilités

• Participate in program development, evaluation, and improvement initiatives.
• Coordinate patient activities and ensure appropriate utilization of healthcare services.
• Engage patients and families, promoting self-management and shared decision-making.
• Develop care plans with providers to maximize health outcomes and cost-effective care.
• Coordinate transitions across the continuum and connect to community resources.
• Educate patients and caregivers to improve health literacy and informed decisions.
• Document care coordination activities accurately and timely.
