Job summary

Quality Healthcare Development in Charleston, SC is seeking a Nurse Care Coordinator to develop and implement a comprehensive care coordination program in partnership with the Quality Director. The role focuses on engaging and supporting patients within the Accountable Care Organization to improve care quality, outcomes, and efficiency.

Qualifications

• Bachelor's degree in Nursing (BSN) required.
• Current RN license in South Carolina required.
• Minimum of five (5) years in a clinical/outpatient setting preferred.
• Experience in care coordination, population health, or value-based care preferred.
• Proficiency with EMR systems.

Responsibilities

• Participate in program development, evaluation, and improvement initiatives.
• Perform patient outreach via telephone and occasional office encounters.
• Maintain regular communication with patients in the assigned caseload.
• Coordinate patient activities and ensure appropriate utilization of healthcare services.
• Support care gap closure and ensure appropriate follow-up care.
• Include patients' family members and caregivers in coordination activities as appropriate.
• Promote patient engagement, self-management, and shared decision-making.
• Develop care plans with providers to maximize health outcomes and cost-effective care.
• Coordinate care across the continuum including post-discharge follow-up calls.
• Assist patients in accessing community resources and financial assistance.
• Identify and address barriers to treatment plan adherence.
• Utilize motivational interviewing and other strategies to promote engagement and health literacy.
• Provide education and resources for informed healthcare decisions.
• Document care coordination activities accurately and timely.
• Serve as a patient advocate throughout the process.
• Maintain HIPAA compliance and awareness of diversity in patient population.
