Quality Healthcare Development in Charleston, SC is seeking a Nurse Care Coordinator to develop and implement a comprehensive care coordination program within the ACO population, improving care quality and patient outcomes while reducing costs.

You will work with patients, families, providers and the health care team to coordinate services, promote engagement and health literacy, and ensure seamless transitions and appropriate follow-up.

Qualifications

• BSN is required.
• Minimum 5 years of clinical/outpatient care experience preferred.
• Current RN license in South Carolina required.
