Job summary

Optum's WellMed Arlington unit seeks an RN HealthCare Manager to support high-risk patients, serving as educator, resource, and advocate to improve quality of life. You will collaborate with physicians, nurses, pharmacists, social workers, and other educators in a less-structured environment while performing nursing duties within RN scope.

The role emphasizes care coordination, patient education, and referrals, with a base in Arlington, TX.

Qualifications

• RN license with active, unrestricted state license.
• BLS certification current and in good standing.
• Experience in physician office, clinic, or hospital setting.
• Knowledge of chronic diseases (COPD, diabetes, CHF, IHD).
• Proficient with electronic medical records.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care: assess health, reconcile meds, educate patients.
• Coordinate transitions from inpatient to outpatient care and update care plans.
• Document accurately in the electronic medical record.
• Participate in PCC meetings and weekly reporting.
• Maintain current nursing practice skills and evidence-based knowledge.
