Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support patients with high-risk conditions. You will educate, empower, and advocate for patients and families to improve quality of life within a multidisciplinary team.

Responsibilities include coordinating care plans, documenting in EMR, and leading care coordination meetings. The role requires an active RN license, BLS, and experience in clinical settings.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience as an RN
• Registered Nurse with an active and unrestricted license to practice in the state of employment
• Current BLS certification
• 2+ years of experience in a physician's office, clinical or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
• Excellent verbal and written skills
• Solid interpersonal skills
• Ability to interact productively with individuals and with multidisciplinary teams
• Proficient computer skills to work efficiently with electronic medical records
• Excellent organizational and prioritization skills

Responsibilities

• Works with providers and clinic staff to identify patients at high risk
• Supports longitudinal care of patients with chronic conditions: assess health, reconcile meds, motivational interviewing, patient education, referrals
• Assists transition of chronic patients from inpatient to outpatient, assess needs, reconcile meds, plan contingencies, educate patients
• Coordinate with providers to establish or update plan of care
• Performs accurate documentation in the EMR
• Participates in daily huddles and PCC meetings
• Prepares reports for weekly meetings
• Maintains competence in nursing practice and evidence-based care
• Performs other related duties as assigned
