Job summary

Requirements include a Bachelor’s degree in Nursing (or an ASN with 4+ years RN experience), active RN license, and current BLS certification.

Qualifications

• Bachelor's degree in Nursing or ASN with 4+ years RN experience.
• Active RN license to practice in the state of employment.
• Current BLS certification.
• 2+ years in physician's office, clinical or hospital setting.
• Knowledge of chronic diseases ( COPD/asthma, diabetes, CHF, IHD).
• Excellent verbal and written skills.
• Solid interpersonal and teamwork skills.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care through assessment, medication reconciliation, MI, education, referrals.
• Coordinate transitions from inpatient to outpatient care and adapt care plans.
• Document accurately in electronic medical records.
• Participate in PCC meetings and weekly reporting.
• Maintain current practice competence and knowledge of evidence-based care.
• Perform other related duties as assigned.
