Job summary

WellMed is seeking a Healthcare Manager to support patients with high-risk conditions, acting as educator, resource, and advocate to improve quality of life. You will collaborate with physicians, nurses, pharmacists, and social workers to coordinate care and ensure smooth transitions from inpatient to outpatient care.

The role requires RN licensure in the state, Bachelor's or Associate's with experience, and BLS certification.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of RN experience.
• Active RN license in state of employment.
• Current BLS certification.
• 2+ years in physician's office, clinical or hospital setting.
• Knowledge of chronic diseases, COPD/asthma, diabetes, CHF, IHD.

Responsibilities

• Identify high-risk patients and support their healthcare navigation.
• Coordinate care plans with multidisciplinary teams.
• Document in EMR accurately and timely.
• Support transition from inpatient to outpatient settings.
• Participate in PCC meetings and weekly reports.
