Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure discharge planning meets goals.

Collaborate with the utilization review nurse, physicians and the care team to optimize progression, address barriers, and promote efficient resource use; strong communication is essential.

Qualifications

• Associates or diploma in nursing; RN license required.
• Bachelor's degree in nursing preferred.
• Experience in acute care and care coordination is advantageous.

Responsibilities

• Assess transitional care needs and psychosocial factors.
• Coordinate care across the continuum and discharge planning.
• Collaborate with physicians, UM nurse, PAS and care team.
• Document interactions and progress in medical records.
• Participate in interdisciplinary rounds and patient education.
