Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure care needs are met. The role manages length of stay, promotes efficient resource use, and integrates care progression with discharge planning.

Collaboration with the care team and physicians is essential to identify barriers and facilitate timely discharge.

Responsibilities

• Assess patients' transitional care needs and psychosocial risk factors.
• Coordinate discharge planning and care progression with care team.
• Collaborate with physicians, PAS, financial counselor and UM nurse to review insurance needs.
• Document care plans and progress in medical records.
• Lead or participate in interdisciplinary rounds and family/patient education.
