Qualifications

• Associates Nursing or Diploma (Nurse) nursing degree required, Bachelor's preferred.
• RN licensure and current BLS or equivalent required.
• Minimum 1 year nursing experience in acute care setting.
• Excellent communication and collaboration with care team and patients/families.

Responsibilities

• Assess patients' conditions and psychosocial needs on admission and during stay.
• Coordinate discharge planning and post-acute referrals with care team and family.
• Collaborate with physicians, UM, PAS and financial counselor to ensure appropriate care and insurance requirements.
• Document assessments, care progression and discharge plans clearly in medical records.
• Participate in interdisciplinary rounds and ensure timely discharge for transitions of care.
