Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate care across the continuum, and engage with patient and family to assure care needs are met. The CC RN plans effectively to meet the patient's needs and promote efficient utilization of resources.

Collaborates with physicians, the care team, and other departments to provide a comprehensive assessment, supports discharge goals, and facilitates post-acute referrals as required.

Qualifications

• Excellent written and verbal communication; able to work under stress.
• Strong assessment, interviewing, organizational and problem-solving skills.
• Knowledge of local, state and federal regulations and resources.
• Ability to collaborate with physicians and care team to progress care.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Discharge planning with patient/family and care team.
• Participate in interdisciplinary rounds and patient education.
• Identify barriers and facilitate efficient progression of care.
