Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care goals are met. The role emphasizes discharge planning, interdisciplinary collaboration, and efficient resource utilization in an acute care setting.

Qualifications

• Excellent written and verbal communication skills.
• Strong assessment, interview, organizational and problem-solving abilities.
• Knowledge of local, state and federal regulations for patient discharge.
• Ability to work collaboratively with physicians, care team, patient and family.

Responsibilities

• Initiate assessment of patients' chronic disease management and psychosocial needs.
• Collaborate with insurance and UM staff to verify coverage requirements.
• Develop and support individualized discharge plans in coordination with physicians.
